What is Care4Neighbors?                    

Care4 is a FREE healthcare savings program that entitles cardholders to significant discounts on dental, vision, prescription drugs, chiropractic and other ancillary healthcare services.

How is Care4Neighbors FREE, when other plans charge $30+ per month?

Care4Neighbors provides discounted healthcare access for FREE because the healthcare providers in our Network discount their services to Members directly, without a managed care or insurance company in the middle. These providers also help pay for the administration of the Care4Neighbors program as part of their participation fees. You, as a Member, never pay anything to participate in the great savings and access provided through Care4Neighbors.    

What providers are in my area?                   

You can search for providers once you have enrolled in Care4Neighbors and have been issued a Member Number.  If you are a Member, go to the Provider Search section of this website or call 800-444-0650 to find providers in your area.  Remember, enrolling is easy, there are no obligations, no contracts and you will never pay to be a Member.  Enroll today.

Do I have to change doctors, dentist or other providers?                

If your current provider is in the network, you only need to show your Membership Card to start saving money.  If your provider is not in our network, you can either call the toll free customer service number on the back of your card to refer your provider or you can choose another provider in the area.  There are no referrals necessary to see a participating provider.

How does the program work?                   

Upon receiving your Membership Card, you can use your Care4Neighbors Card at any participating Network Provider by simply presenting your Card at the time of service. You will be charged the discounted fee for your services at that time.

How is the program different from insurance?                   

Care4Neighbors is different than insurance because there are no restrictions on pre-existing conditions, no state exclusions, no age restrictions. This is a discount plan, so we do not reimburse you or your doctor for medical expenses. You are responsible for paying the discounted fee at the time of service.

Do I have to wait for my Card in the mail to receive discounts?                   

No.  If you enroll here, you will receive a Member Number and a temporary Card to print. You can start using that Card immediately.  Your permanent Card will be mailed to you in 2 - 3 weeks. If you have any problems with a provider or pharmacy accepting your temporary Card, please call us at 800-444-0650. 

If I have health insurance, how does this plans work with my existing plan?                   

Care4Neighbors can be used along with your medical insurance to reduce your out-of-pocket expenses.  Plus, the discount card can apply to services not covered by your medical insurance.  In many circumstances, dental, vision, chiropractic and prescription drugs are not covered by medical insurance plans. Care4Neighbors also complements Health Savings Accounts and high deductible plans.

What if I have a question or need any help?

Are there any restrictions on age or pre-existing medical conditions?                   

No, since Care4Neighbors is not an insurance program there are no restrictions on age or pre-existing medical problems.

Can membership be denied?                   

No, your membership cannot be denied due to age, income or pre-existing conditions. However, spaces are limited during our Enrollment Periods, and we fill applications on a first-come first-served basis. 

Can anyone in my immediate family use my membership card?                   

Yes, immediate family members residing in your household can use your Care4Neighbors Membership Card. Make sure to list their names and birth dates with enrolling for your Card.

If I get married will my spouse be covered?                   

Yes.

Can I get a second card for my spouse, child, etc.?                   

Yes, but there is a shipping and handling charge of $4.95.

What if I lose my card?                   

If you lose your card, please call 800-444-0650 so a new one can be sent to you. There is a shipping and handling charge of $4.95.

 

Will this membership keep my medical information private?                   

Care4Neighbors has no access to private medical information, only the providers have that information.

Do I have to pay at the time of service?                   

Yes, you must pay the provider once you've received the service. Participating providers have an agreement that they will not bill the member for the difference between their usual fee and the reduced fee.

How is pricing for prescription drugs determined?                   

The Care4Neighbors prescription drug program provides savings on both generic and name-brand prescriptions. It allows for unlimited use by the member and his/her family. Prescription drug costs vary by provider due to differences in both retail and wholesale prices for their products. When the pharmacy’s usual and customary charge (retail price) is lower than the Care4Neighbors discounted price, members will pay the lowest price. 

Does the Care4Neighbors Card cover co-payments?                   

No, Care4Neighors is not insurance. The plan gives you discounts at healthcare providers in your area.  

What if I need a provider in another community or state?                   

Right now, Care4Neighbors is only available in South Florida.

What if a provider doesn't accept my card?                   

If a provider does not accept the card, contact customer service at 800-444-0650 and the representative will contact the provider and mediate the situation.

What if my provider is not a listed participant in your program?                   

On this site you can check providers in your area. You must use a healthcare provider within the network for discounts to apply.

With your Care4Neighbors card you can access all member benefits and discounts. To receive discounts, present your Membership Card to the provider when you arrive for your discounted product or service. He/She will find out the reduced fee for the services given using your Member number. You must pay the reduced fee at the time of service. Participating providers have an agreement that they will not bill the member for the difference between their usual fee and the reduced fee.
