Requirements for Providers

Qualifications

Participating Providers must:

Be fully licensed and in good standing in their applicable jurisdiction
Hold a valid DEA certificate, and other anesthesia permits, if applicable
• Be free from past disciplinary actions, malpractice convictions or other sanctions  

Participation Fees

Providers pay fees to the Network that help pay for administration of Care4Neighbors and communication to new and future Members.  Those fees are determined in part by:

• Whether you are a Primary Provider or Specialist,
• The number of offices you operate, and
• The geographic scope of your Network listings.

Networks are broken down by Zip Codes to allow our Providers the ability to reach and help families in their immediate neighborhoods. Provider fees are small, and generally no more than yellow pages listings or community flyer/circular ad rates.  When you request more information, a representative will provide you with your exact participation fees.

Discount Provided Patients

Providers agree to discount their fees to registered, cardholding Members. These discounts vary based on your field of practice and the location of your office. Generally, however, discounts range between 15% and 25% off Usual & Customary Rates.
